- I give permission for my child to participate fully in all activities associated with the 2023 Believe Church Youth Camp, unless I have advised the Believe Youth leaders in writing otherwise.

- I give permission for my child to travel in a car driven by an approved Believe Youth leader or approved parent, unless I have advised the Believe Youth leaders in writing otherwise. (Your child will not be in a car driven by a learner or a provisions licence holder). I give permission for my child to travel by bus or minibus.

- I authorise the Believe Youth leaders in the event of an emergency, to obtain at my expense, any medical, ambulance, rescue or other services that are considered necessary for my child.

- I acknowledge that being part of a community involves mutual care and consideration, and therefore agree that unacceptable behaviour by my child, will result in me being required to collect my child from camp and it may result in them being temporarily or permanently restricted from attending future Believe Youth activities. I understand that my child's camp registration fees will not be refundable.

- I will provide the Believe Youth leaders with any information relevant to the wellbeing of my child prior to him or her attending the 2023 Youth Camp.

- I give consent for photos and videos of my child participating in activities, to be taken throughout the camp, understanding that these may be displayed publically and on social media, unless I advise the Believe Youth leaders in writing otherwise.

- I give consent to be contacted by Believe Youth leaders by phone or email, unless I advise the Believe Youth leaders otherwise.

- I confirm that the information I have provded on the form is true and correct, and will advise the Believe Youth leaders of any changes to this information before camp.

- I acknowledge the requirements to have completed the Believe Church Volunteer Checklist prior to attending 2023 Youth Camp as an adult/leader.

- I understand that if I have not completed this checklist prior to the start of camp, that my camp registration fees will be forfeited and are not refundable.

- I agree to behave in a manner fit for a Believe Youth leader at all times, prioritising the safety and wellbeing of all attendees, but especially children, throughout the camp.

- I authorise the Believe Youth leaders in the event of an emergency, to obtain at my expense, any medical, ambulance, rescue or other services that are considered necessary for my own wellbeing.

- I acknowledge that being part of a community involves mutual care and consideration, and therefore agree that any unacceptable behaviour, will result in me being required to leave the campgrounds at my own expense, and it may result in me being temporarily or permanently restricted from attending future Believe Youth activities. I understand that my registration fees will not be refundable.
